Services are provided within an agreed scope. Valentine Care Collective is a private healthcare-navigation and care-management service, not a home-care staffing agency.
01 / Support
Initial Consultation
An approximately 90-minute structured verbal review of the current situation, immediate concerns, care needs, existing supports, gaps, priorities and next steps.
Consultations may take place virtually, by phone, in a retirement setting or in a hospital setting where appropriate. A standard in-home consultation is not currently offered.
A written summary is not automatically included. Written planning products are separate where applicable.

Valentine Care Roadmap
A comprehensive written planning product that brings the broader care picture into focus.
It may include priorities, identified gaps, support options, referrals to consider, professionals or services to involve, implementation steps, family responsibilities, follow-up, monitoring and contingency considerations where appropriate.
03 / Support
Private Family Care Management
Depending on the agreed arrangement, support may include guidance, coordination, follow-up, organization and communication with authorized family members and providers.
We help families track changing care needs and understand next steps. The scope and availability of support are established in the service arrangement.

Aging-at-Home Planning
Explore the supports, priorities and practical considerations involved in aging at home.
We help families review what is already in place, recognize gaps and organize options and next steps around changing needs.
05 / Support
Hospital-to-Home Transition
Guidance for families navigating the move from hospital to home, including understanding next steps, relevant supports and the professionals who may need to be involved.
Coordination is provided where included in the agreed service arrangement.

Dementia Family Support
Help for individuals living with dementia and their families to understand the broader care situation, explore support options and organize priorities.
We approach care conversations with dignity, attention to family capacity and respect for the individual’s involvement.

Facility Transition Support
Support for families considering or navigating a move to a care setting. We help organize questions, explore options and clarify the practical next steps involved in a transition.

Monitoring, education & speaking

Scheduled reviews, follow-up and reassessment of changing needs, current supports and emerging gaps. In-person visits or assessments are included only where appropriate and expressly agreed in the service arrangement.
09 — Education & Professional Speaking
Inquiries are welcome from families, professionals and organizations. A defined education catalogue is in development.
