Karliyah’s professional background includes acute-care nursing, seniors and retirement care, memory care, dementia support, care coordination, healthcare leadership and family education.
That background informs a thoughtful, practical approach to helping families understand the broader care picture: what is already in place, where gaps may exist and which next steps deserve attention.
The purpose behind Valentine Care Collective
Care can involve many people, appointments, recommendations and services without one clear path connecting them. Valentine Care Collective helps individuals and families understand the broader care picture, recognize gaps and organize a clearer way forward.
Our starting point is the individual: their preferences, routines, relationships, current abilities and the support already around them. We also consider the practical capacity of family members and caregivers.
The purpose is to bring clarity and organization to care decisions while keeping the person at the centre of the conversation.
A considered scope of support
Support may begin with an approximately 90-minute structured verbal consultation to discuss the current situation, concerns, existing supports, priorities and possible next steps. A written summary is not automatically included.
The Valentine Care Roadmap is a separate comprehensive written planning service. Private Family Care Management may include guidance, organization, communication, coordination and follow-up, depending on the agreed arrangement.
Scope and availability are established before services proceed. Valentine Care Collective is a private healthcare-navigation and care-management service, not a home-care staffing agency.
